ABSTRACT

OBJECTIVE@#To improve the electromagnetic compatibility (EMC) quality of medical devices, improve the efficiency of EMC testing, and promote the speed of market approval.@*METHODS@#The unqualified cases of EMC test items of medical devices in recent years were statistically analyzed, and the reasons of low EMC quality of medical devices were analyzed from the perspective of test.@*RESULTS@#Based on the analysis of the reasons, the suggestions were given from the perspectives of medical device manufacturers and testing organizations.@*CONCLUSIONS@#In order to ensure the quality of EMC of medical devices, medical device manufacturers, regulatory authorities and inspection and testing institutions should strengthen the monitoring and evaluation of medical device electromagnetic compatibility, to ensure the safety of products work together to promote the development of the medical device industry healthily and orderly.

ABSTRACT

A case of familial hypocalciuric hypercalcemia type 1 (FHH1) was reported detailing the course of diagnosis and treatment. The main clinical manifestations of the patient were recurrent pancreatitis with moderate hypercalcemia and low urinary calcium. The C→T heterozygous missense mutation at nucleotide 2 393 with conversion of codon Pro798 to Leu (p.P155L) in CaSR gene was identified. Serum calcium and parathyroid hormone levels of the patient were decreased significantly after treatment with cinacalcet.

ABSTRACT

Objective:To investigate the effects and potential mechanism of vitamin D supplementation on testicular function in aging rats induced by D-galactose.Methods:The aging rats were induced by D-galactose with subcutaneous injection. The animals were randomly divided into 6 groups: aging rats (DG), aging rats with low-dose vitamin D supplementation (LD), aging rats with high-dose vitamin D supplementation (HD), normal control rats(NC), normal rats with low-dose vitamin D supplementation(LN), normal rats with high-dose vitamin D supplementation (HN). The body weight, testicular weight, serum testosterone concentrations and sperm quality of the rats in each group were measured. The testis morphological changes were detected using light microscopy. The activity of superoxide dismutase (SOD) and level of malondialdehyde(MDA) were determined with spectrophotometer. The expression levels of Nrf2, GCLC, SOD2 and VDR in testis were detected by western blot.Results:At baseline, compared with NC group, testicular weight, serum testosterone level, SOD activity, Nrf2, GCLC and SOD2 expression levels were significantly decreased in DG group, while MDA level was significantly increased. After vitamin D supplementation, testicular weight, testosterone levels and SOD activity in both of HD and LD groups were significantly increased, while the MDA level was significantly decreased. The expression levels of Nrf2, GCLC, SOD2 and VDR were significantly increased.Conclusion:Vitamin D supplementation may enhance the testicular antioxidant capacity through activating Nrf2-ARE signaling pathway, and improve the testicular function in D-galactose-induced aging rats.

ABSTRACT

Objective:To investigate the effect of miR-425-5p on glucagon-like peptide-1(GLP-1) secretion in intestinal L cells induced by lipopolysaccharide(LPS), and to explore its mechanism.Methods:GLUTag cells of intestinal L cell line were incubated with LPS to determine the levels of miR-425-5p and GLP-1. Cell viability was determined by MTT assay, and cell apoptosis was detected by flow cytometry. Quantitative real time-PCR and western blot were performed to determine the expressions of miR-425-5p, phosphatase and tensin homology(PTEN), proglucagon, and GLP-1. Activity of Wnt/β-catenin signaling pathway was determined by detecting TOP/FOP ratio. Interaction among miR-425-5p, PTEN, and β-catenin was analyzed using luciferase activity assay and chromatin immunoprecipitation(ChIP)assay.Results:In GLUTag cells, with the elevation of LPS concentration, the expression of miR-425-5p and the apoptosis rate were increased, while the level of active GLP-1 and the cell viability were decreased. MiR-425-5p was involved in the regulation of LPS on GLP-1 secretion and intestinal L cell viability. Inhibition of miR-425-5p reduced the mRNA expression of proglucagon and the TOP/FOP ratio, increased PTEN protein level, and inhibited cell viability. In LPS-treated GLUTag cells, miR-425-5p increased the level of β-catenin by targeting PTEN, and β-catenin acted as a cis-acting element to induce the transcription of proglucagon and promote the secretion of GLP-1.Conclusion:In LPS-induced intestinal L cells, miR-425-5p promotes the expression of GLP-1 by targeting PTEN to modulate β-catenin.

ABSTRACT

Obstructive sleep apnea hypopnea syndrome(OSAHS)is a common sleep and respiratory disorder, characterized by periodic collapse of the upper airway totally or partly during sleep, disrupting normal ventilation and sleep structure, and causing a series of pathophysiological changes.Recently, the incidence of obesity among children increases year by year with the improvement of living standards and the change of lifestyle, and severely affects children′s physical and mental health.At the same time, the prevalence of OSAHS is also increasing, which can result in serious adverse outcome.A large amount of research has shown the close relationship between obesity and OSAHS.There is a certain correlation in the pathogenesis, making the two influence each other in the treatment.To provide reference for clinical practice, this review mainly discusses the correlation between childhood obesity and OSAHS in terms of pathogenesis and treatment.

ABSTRACT

Objective:To examine associations of sex hormone levels with bone turnover markers(BTMs) among men in the Northeast region of Henan Province.Methods:From December 2015 to March 2016, 707 male subjects were selected from a National Epidemiological Survey-2014(Thyroid Disorders, Iodine status and Diabetes, TIDE)research—Henan sub-center survey by using multistage stratified cluster random sampling. Fasting venous blood was collected to determine the levels of luteinizing hormone(LH), follicle stimulating hormone(FSH), estradiol(E 2), testosterone(T), dehydroepiandrosterone-sulfate (DHEAS), androstenedione(AD), sex hormone binding globulin(SHBG), dihydrotestosterone(DHT), free testosterone(FT), osteocalcin(OC), pro-collagen type 1 N-terminal propeptide(PINP), C-terminal-cross-linking telopeptide of type 1 collagen(β-CTX), 25-hydroxyvitamin D [25(OH)D], and parathyroid hormone(PTH). Results:A total of 697 men with an average age of(46.6±15.9)years were included in the study. Pearson correlation analysis showed that age was positively associated with LH, FSH, T, and SHBG, while negatively associated with E 2, DHEAS, AD, FT, β-CTX, OC, and PINP, without significant correlation with DHT, 25(OH)D, and PTH. Pearson correlation analysis and linear regression analysis showed that E 2 was negatively associated with β-CTX; T was positively associated with OC, FSH was negatively associated with OC; LH, FSH, and SHBG were negatively associated with PINP; E 2, T, FT, DHT, and AD were positively associated with PINP. After adjusting for age and BMI, linear regression analysis showed that T was still significantly positively associated with OC and PINP, with 0.302 ng/ml and 0.015 ng/ml increasing for OC and PINP every 1 ng/ml increase in T; E 2 and DHT were positively associated with PINP, with 0.250 and 0.047 ng/ml increasing for PINP every 1 pg/ml increase in E 2 and DHT. Conclusions:Age is an important factor influencing sex hormones and BTMs. Serum levels of T, E 2, and DHT are associated with bone formation and bone absorption markers.

ABSTRACT

The study was initiated from a child with diabetes. After we collected his clinical data and traced back his family history of diabetes, a clinical diagnosis of maturity-onset diabetes of the young(MODY)was made. To amplify and sequence the target gene, the genomic DNA was extracted from the anticoagulant blood samples of the patient and his first-degree relatives, revealing a missense mutation(c.779C>T)in exon 4 of hepatocyte nuclear factor-1α in the proband and his father. The above sequencing result confirms the diagnosis of MODY3. During one year follow-up, the proband achieved the strict control of blood glucose with the use of repaglinide and his father got a notable improvement of blood glucose after his drug was shifted to the long-acting sulfonylurea.

ABSTRACT

OBJECTIVE: To investigate the current status of emotional intelligence and perceived professional benefits in undergraduate nursing student interns(hereinafter referred to as nursing student intern) and to analyze their correlation. METHODS: By convenience sampling method, 322 nursing student interns from 3 class A general hospitals in Nanchang City were selected as study subjects. Their emotional intelligence and the occupation benefits they obtained were investigated by Emotional Intelligence Scale and Nurses′ Perceived Professional Benefits Scale. RESULTS: The total average score of emotional intelligence was(3.6±0.6) and the average scores of each dimension item were others′ emotional assessment(3.8±0.6), self-emotional management(3.6±0.7), self-emotional use(3.6±0.7) and self-emotional assessment(3.5±0.7). The total average score of perceived professional benefits was(3.9±0.6) and the average scores of all dimensions were good nurse patient relationship(4.1±0.6), self growth(4.0±0.7), team belonging(3.9±0.6), family identity(3.9±0.6), and positive occupational perception(3.7±0.7). Emotional intelligence and its four dimensions were positively correlated with nursing perceived professional benefits and its five dimensions(P<0.01). Hierarchical multiple regression analysis results showed that, emotional assessment of others and self-emotion management had a positive predictive effect on perceived professional benefits, and explained 41.5% of the variance. CONCLUSION: The emotional intelligence of nursing student interns and their perceived professional benefits were positively correlated. Nursing educators could improve the perceived professional benefits of nursing student interns by strengthening the training of emotional intelligence.

After fully adjusting for age, residence area, economic status, education, body mass index, waist circumference, homeostasis model assessment of insulin resistance (HOMA-IR), hypertension, diabetes, triglyceride, high-density lipoproteincholesterol, total cholesterol, low-density lipoprotein cholesterol and uric acid, linear regression analyses showed that every 25 nmol/L increase in 25(OH)D levels increased lg FT(FT=free testosterone) by 0.013ng/L (β=0.013, P=0.036), lg DHT (DHT=dihydrotestosterone) by 0.030 ng/L (β=0.030, P=0.019), and lg AD (AD=androstenedione) by 0.019 μg/L (β=0.019, P=0.008). After fully adjusting for age, residence area, economic status and education, every 25 nmol/L increase in 25(OH)D levels lowered glycosylated hemoglobin A1c (HbA1c) by 0.051% (β=-0.051, P=0.027).@*Conclusions@#Higher 25(OH)D concentrations in men were associated with higher FT, DHT, AD and lower HbA1c levels.

ABSTRACT

Objective To explore the effects of modified herringbone-Trendelenburg position in the laparoscopic anterior resection for rectal cancer patients. Methods A total of 108 patients undergoing laparoscopic anterior resection were recruited and randomly assigned to observation group(54 cases) and control group (54 cases). Patients in the observation group were positioned in modified herringbone-Trendelenburg position, while the patients in the control group were placed in conventional Trendelenburg position. The data of heart rate (HR), mean arterial pressure (MAP) were recorded at 3minutes before body position change and 3 minutes after body position change, 3 minutes before recover horizontal position and 3 minutes after recover horizontal position. Intraocular pressure (IOP) were measured at 3 minutes after general anesthesia in supine position (T0), and 3 minutes after pneumoperitoneum while in the operation position (T1), every 1 hour (T2 to T3), 3 minutes before recover horizontal position at the end of pneumoperitoneum (T4), 3 minutes after recover horizontal position (T5) and 30 minutes after recover horizontal position(T6). Investigate the satisfaction of the surgeons regarding the surgical position of the patients.Followed up investigation at 24h and 48h after surgery were enforced to record the situation of the pain in the shoulder and postoperative complications of the lower limbs. Results The heart rate pre-and post the change of body position in observation group were (2.11±0.92), (-2.78±1.01) beats/min respectively, while the control group were (5.98±2.98), (-6.03±1.98) beats/min, the differences were statically significant (t=9.111,9.851, P<0.01).The mean arterial pressure pre-and post the change of body position in observation group were (1.67 ± 1.23), (2.21 ± 0.89) mmHg(1mmHg=0.133kPa) respectively, while the control group were (7.20±2.30), (6.41±1.87)mmHg, the differences were statically significant(t=15.512, 14.811, P<0.01).The differences of intra-ocular pressure between 2 groups had no statistical significance at T0 and T6 (P>0.05). The intra-ocular pressure were (13.64±1.66), (16.56±1.82),(19.78±1.70),(21.00±1.71),(18.53±1.77)mmHg respectively from T1 to T5, lower than that of control group (15.59±2.03),(19.40±1.89), (23.22±2.15), (25.38±2.09), (22.35±1.76)mmHg, the differences were statically significant (t=5.442-11.907, P<0.01).The incidence of shoulder pain and low leg pain in observation group were 9.26%(5/54), 7.41%(4/54) respectively, lower than that of control group 46.29%(25/54),31.48%(17/54), the differences were statically significant (t=17.778,9.755,P<0.01). The scores of shoulder pain and low leg pain in observation group were (1.38±0.38), (2.02±0.34) points, lower than that of control group (4.44 ± 0.48), (3.85 ± 0.57) points, the differences were statically significant (t=36.761, 20.162, P<0.01). The satisfaction rate of surgeons was 87.04%(47/54) in the observation group, higher than that in the control group 55.56% (30/54), the difference was statically significant (χ2=5.119, P=0.024). Conclusion Modified herringbone-Trendelenburg position can maintain the circulatory system stability better without affecting the operation, reduce the elevation of IOP, effectively improve the comfort of the operation position of the patients,reduce the postoperative complications of the lower limbs and the incidence rate of the shoulder pain in the laparoscopic anterior resection for rectal cancer patients.

ABSTRACT

Recently, a novel atypical porcine pestivirus (APPV) in pig was reported. In this study, two APPV strains, APPV-China/GZ01/2016 (GZ01) and APPV-China/GD-SD/2016 (GD-SD), were identified in two newborn piglet herds with congenital tremor from China. The open reading frame of the two strains shared an 83.5% nucleotide identity. Phylogenetically, the APPV strains were placed into two groups: GZ01 belonged to group I and GD-SD belonged to group II. A high viral load was detected in the cerebellum (quantification cycles < 26). Further studies should be carried out to thoroughly elucidate the development of congenital tremors caused by APPV.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of the combined therapy of heat sensitive moxibustion and acupoint injection on endometrial receptivity of hypdrosalphinx infertility in the patients after hysteroscopy and laparoscopy on the base of routine post-operative anti-inflammation.</p><p><b>METHODS</b>A total of 210 patients of hypdrosalphinx infertility after hysteroscopy and laparoscopy were divided into a combined therapy group, a heat sensitive moxibustion group and a control group, 70 cases in each one according to the random number table. In the control group, the intravenous drip of cefoxitin sodium was adopted, and the anti-inflammation treatment was given for 1 week after operation. In the heat sensitive moxibustion group, on the basis of the treatment as the control group, the heat sensitive moxibustion was applied after vaginal bleeding stopped. The acupoints were Yaoyangguan (GV 3), Guanyuan (CV 4), Qihai (CV 6), Shenshu (BL 23), Sanyinjiao (SP 6), Yinlingquan (SP 9) and Zigong (EX-CA1). The acupoints were modified according to the different syndromes. In the combined therapy group, on the basis of the regimen as the heat sensitive moxibustion group, after vaginal bleeding stopped, the acupoint injection was given alternatively at bilateral Tiangong (extra, 1.0 cm inferior and bilateral to the cervix) with lidocaine 1 mL, amikacin 2 mL and salvia injection 2 mL. The treatment was given once every day, the treatment for 10 times as one course and a total of 3 courses were required. The endometrial type, thickness, uterine arterial plusatility index (PI) and resistance index (RI) were observed in the patients of each group.</p><p><b>RESULTS</b>After treatment, the numbers of A-type endometrial type in the combined therapy group and the heat sensitive moxibustion group were remarkably higher than those of the control group [57.1% (40/7) vs 31.4% (22/70), 50.0% (35/70) vs 31.4% (22/70), both<0.05]. The endometrial thickness after treatment was all increased as compared with that before treatment in each group (all<0.05). The increasing degree in the combined therapy group was better than either the heat sensitive moxibustion group or the control group (both<0.05). The improvement in the heat sensitive moxibustion group was superior to the control group (<0.05). PI and RI in the combination group and PI in the control group were decreased after treatment (all<0.05). The improvements of PI and RI in the combination group were better than those in the heat moxibustion group (both<0.05), which were superior to those in the control group (all<0.05).</p><p><b>CONCLUSION</b>In the patients of hypdrosalphinx infertility after hysteroscopy and laparoscopy, the combined therapy of heat sensitive moxibustion and acupoint injection increases endometrial thickness, reduces uterine arterial resistance and improves endometrial receptivity.</p>

ABSTRACT

Objective To optimize the lateral position of the operation,to improve the comfort of the lateral decubitus patients, to expose the incision adequately, and to improve the nursing quality of lateral decubitus surgery. Methods A total of 60 cases of thoracic surgery patients with lateral decubitus were divided into the control group and the test group according to the random number table method, 30 cases each. The control group used traditional lateral decubitus placement method, experimental group through the method and fixed on the traditional method of placing lateral limbs was improved, used in thoracic surgery especially in thoracoscopic surgery. The effect was compared between two groups. Results The time between the experimental group and the control group was respectively (5.6 ± 0.7), (9.0 ± 0.5) min, and the difference between the two groups was statistically significant (t = 26.944, P <0.01). In the two groups, the muscle soreness of 24 – 48 h after operation was 1 and 12 cases, and 2 groups were statistically significant(χ2=13.35,P<0.01).The postoperative satisfaction of the two groups of surgeons, anesthesiologists and nurses was statistically significant (Z =- 2.573, P < 0.01). Conclusions The modified lateral application in thoracic surgery is superior than the traditional lateral position, lateral position display method of time and labor saving, energy saving, expand the operation field,improve patient comfort,reduce costs,and improve the safety of nursing quality in operation room.

ABSTRACT

OBJECTIVE:To study the improvement effect of geniposide on cognitive dysfunction in model rats with diabetes. METHODS:50 rats were randomly divided into blank control group(distilled water),model group(distilled water)and genipo-side low-dose,medium-dose,high-dose groups(5,10,20 g/kg),10 in each group. Except for blank control group,rats in other 4 groups were fed with high-sugar and high-fat diet and injected streptozotocin to induce diabetes model. After modeling,rats in each group intragastrically administrated relevant medicines,once a day,for 8 weeks. After last administration,fasting blood glu-cose(FPG),glycosylated hemoglobin(HbA1c)levels,escape latency period in Morris water maze test,and correct rate of behav-iors in Y maze test of rats in each group were detected. RESULTS:Compared with blank control group,FPG,HbA1c levels of rats in model group were obviously increased,escape latency period was obviously prolonged,correct rate of behaviors in Y maze test was obviously decreased,with statistical significances(P<0.05). Compared with model group,above-mentioned indexes in genipo-side groups were obviously improved(P<0.05),which were positively correlated with dose(P<0.05). CONCLUSIONS:Genipo-side can effectively reduce blood glucose level of model rats with diabetes and improve their cognitive dysfunction.

ABSTRACT

Objective To investigate the bacteriostatic effect of on the surface of bacter contamination survey sterile protective during microscope operating, to provide basis for the implementation of the preventive hospital infection. Methods Routine surgical care was used as the control group(100 cases). The intervention group(100 cases) was carried out during neurosurgery performed between February and December 2016. Instrument nurses wiped the microscope aseptically with the blood by using new sterile injection water wipes with non blood mucus. The Results of bacterial monitoring and the incidence of nosocomial infection were compared before and after the intervention. Results There was no significant difference between two groups during the former four hours of the neurosurgery(P>0.05). Respectively after using microscope 5 h and 6 h, the qualified rate of bacteria monitoring on sterile protective sleeve surface were increased to 97/100(no wipe 71/100)and 95/100(no wipe 51/100)in intervention group,the difference were statistically significant(χ2=25.15,49.11,P<0.01). Conclusions The bacteriostatic effect of cleaning blood stain on the surface of sterile protective during microscope operating is significant.The intervention is worthy of promotion in neurosurgery.

ABSTRACT

[Objectives]Observation of thermal moxibustion combined Ondansetron prevention and treatment of the effect of deficiency syndrome nausea and vomiting by malignant tumor chemotherapy,to explore using thermal moxibustion combined Ondansetron method,it can reduce the feasibility of chemotherapy induced gastrointestinal adverse reaction,to further improve the life quality of patients,provide safe effective treatment measures.[Method]60 patients were randomly divided into treatment group(30 cases) and control group(30 cases),two groups of patients had comprehensive chemotherapy regimens,control group adopted Ondansetron conventional anti-nausea,on the basis of the control group,treatment group with thermal moxibustion used Zhongwan,Shenque(CV 8),Neiguan(P6) Zusanli(ST36) and Weishu acupuncture points,and combined the Ondansetron treatment,the course of the treatment for 7 days,respectively,on day 1,day 3,7,record nausea and vomiting and appetite symptoms integral.Using SPSS18.0 software system,analysis and evaluation of the symptoms of different time points before and after treatment the change of the integral.[Results]Compared with control group,treatment group of nausea,vomiting,loss of appetite each score significantly lower than the control group,the difference had statistical significance(P<0.05,P<0.01)).[Conclusion]Thermal moxibustion combined with Ondansetron can effectively reduce the degree of deficiency syndrome nausea and vomiting caused by the malignant tumor chemotherapy,can effectively improve the patients' tolerance to chemotherapy.

ABSTRACT

Objective To analyze the clinical characteristics of two patients with 3β-hydroxysteroid dehydrogenase deficiency and to explore their molecular genetic defects.Methods The clinical features and laboratory data of two patients were collected.The exons of HSD3B2 gene were amplified by PCR and sequenced by Sanger sequencing.Results Patient 1, aged 5 yrs old, was raised as a girl with 46, XY karyotype, presented with hyperpigmentation, female infant vulva, clitoral hypertrophy, and bilateral cryptorchidism;Patient 2, aged 11 yrs old, was raised as a girl at birth but as a boy after 1 yr old for known 46, XY karyotype, presented with hyperpigmentation, micropenis and severe hypospadias.Both patients had markedly elevated adrenocorticotropin and decreased cortisol.Two homozygous missense mutations in HSD3B2 gene were identified:conversions of codon Pro155 toLeu(p.P155L)inpatient1,andcodonAla82toThr(p.A82T)inpatient2,bothofwhichwerereportedforthe first time in China.Conclusion The patients with 3β-hydroxysteroid dehydrogenase deficiency in 46,XY karyotype mainly present with male pseudohermaphroditism and adrenocortical deficiency, and the diagnosis should rely on the steroids detection and HSD3B2 gene screening.

ABSTRACT

Objective: To describe the clinical characteristics with long-term prognosis in patients with mid-ventricular obstructive hypertrophic cardiomyopathy (MVOHCM). Methods: A total of 66 MVOHCM patients treated in our hospital were retrospectively studied for their morbidity, clinical characteristics and mortality. The cumulative survival rate was calculated by Kaplan-Meier method; the risk factors for cardiac death and cardiovascular events were analyzed by uni- and multivariate Cox proportional hazard model. Results: There were 66 (2.74%) patients suffering from MVOHCM among 2413 patients of hypertrophic cardiomyopathy and the average diagnostic age was (40.16 ± 14.64) years. With (7.30 ± 6.25) years of follow-up study, the cardiovascular mortality was 13.6% and unexplained syncope (HR=13.37, 95% CI: 1.65-114.46, P=0.015) was the independent predictor for cardiovascular death. There were 45.45% (30/66) patients experienced at least 1 time of cardiovascular event and the most frequent one was non-sustained ventricular tachycardia (NSVT); 19.70% (13/66) of patients combined with apical aneurysms, and they were more inclined to experience NSVT. Conclusion: MVOHCM patients usually have unfavorable prognosis with the higher incidence of cardiovascular events, some patients may develop apical aneurysm. The early diagnosis of MVOHCM is important for appropriate treatment.
